How to prepare for your vaccine:

1. Arrive a few minutes prior to your vaccine in order to fill out the proper forms.

2. Bring your ID, prescription insurance card (if you have one), and vaccination card (if you are coming for your 2nd COVID shot or booster shot). Types of identification that can be presented include a driver’s license, birth certificate, US passport, permanent resident card, Certificate of Naturalization, or consulate ID.

3. Be sure to get plenty of rest the day before. Ensure that you are hydrated and have eaten before the appointment.

4. Remember to wear your mask and preferably a short sleeves shirt for easy administration!

For COVID-19 Vaccine: Once your first shot has been administered you will receive a vaccination card with the location, date, and type of vaccine you received. Keep this card safe as it will be needed for your next appointment. Your next appointment will be scheduled for 3 weeks later (Pfizer vaccine) or for 4 weeks after (Moderna vaccine).

Remember that it takes about two weeks before the vaccine begins to take full effect. Keep wearing a mask, wash your hands and keep a 6 feet distance at all times. Encourage others in your family and community to get vaccinated. Let’s lessen the severity of the virus and stop the spread of COVID-19!
